$18 is paying for time-wasting. Otherwise, people can make an appointment and not turn up and say children are free anyway.
If your child is sick, how can you forget? Also, Free food does not mean you can waste food. Same principle.

Also, every clinic is allocated for about $500 per registered child for their medical fee every year by the government. IF the child goes there very often, some dr will do it free if it already exceeds $500 as some other children do not get sick too often, so it average out
