有些公司的HR部門會要求一封covering letter寫明妳的要求， 預產期，要申請多久的parental leave，leave 開始日期等等。。連同midwife寫的信正明妳的預產期，這兩封信連同IRD的申請表就可以。

Covering letter 例子 -
Dear xxx,
I would like to apply for parental leave.  Please find attached the IRD parental leave application form and a letter from my midwife confirming when my baby is due.  
I am requesting leave from xxxx to xxxx, resuming work on xxxx.  A breakdown of the leave is as detailed in the application form.
Thank you for your support at this time.
