Hi Blah,
It has been a year since I took on the xxxx role. I really enjoy my work and I believe I have done a good job.
I have enjoyed the challenges and hardwork during the year. It has been a good experience to me and I have learned a lot from it. I think we have achieved a lot together and I am looking forward to take on more challenges.
At this moment, I would like to request for a pay rise to reflect my value to th company. I believe my current renumeration does not reflect my increased experience and newly developed skills. I have found out through my research that the current medium market renumeration for a xxxx in Auckland for 2008 is around xxk-xxk.
Based on my track record, I have achieved solid targets during the year xxxx. The company has increased/achieved xxxx with my contribution. Therefore, I believe I deserve a increase of the amount of xxxx per annum.

Generally you don't write a letter to request for a raise. You need to have balls and talk to him/her face to face.
