关于EOI表格填写

小弟准备申请PR, 现在正在填写EOI， 但是遇到点问题， 想请教下
1. Please provide your Birth Certificate number and the name of the issuing authority.
   但是我看了我的出生证明上没有Number啊？ 那这里怎么填写呀
2. Social Security number  这个要填写什么Number呀


3.Driver licence 我现在用的是翻译驾照。难道要我把国内的驾照写上去么？


4. National ID Number, 这个是填写身份证号么？ 如果填了，那是不是到时候给材料就要把身份证公正了啊？
5.Chinese Commercial Code 这个是什么号呀


6.Please list all countries, including all countries of citizenship, you have lived in for a total of 12 months or more in the last 10 years. Include all countries where your stay has been broken by any departures. Use the last entry to include the country in which you are currently living but leave the "To" field blank. 这句话完全不懂啊。。。。

7. Alternatively do you hold a report from the New Zealand Qualifications Authority assessing the level of your qualification?
	PLEASE NOTE: if you have only submitted a Pre-Assessment Result (PAR) for your EOI, and you are invited to apply for residence, the level of your qualification will have to be confirmed through a full International Qualifications Assessment (IQA) by NZQA before points can be awarded. 我学的是IT,我好像没有这个NZQA的东西啊 。 怎么去要这个report啊

求大神们指点下 不胜感激啊~~~

1. 只有出生公证没有出生证这个可以不填 或写 N/A
2. 填身份证号
3. 可以
4.可以，到时候不需要交身份证，护照就够了
5. 这是你名字的标准中文电码，好像没什么用，你可以查了填上
6. 如果这句完全不懂，最好找个移民中介或移民律师咨询。

谢谢，我还有一个问题， 我有个女朋友在国内 那么要在 Section T: Principal Applicant's Other Family
中给写上去么？ 我这次不准备带她一起办，到时候我办好再帮她办， 如果我写了的话，移民局会查么? 谢谢
