1.
To apply to have your overseas vaccination(s) entered into the CIR you will need to send evidence to the Ministry of Health. You will need to send the following information to covid-19overseasvaccinations@health.govt.nz:

your full name (first name, middle name and last name)
your date of birth (DD/MM/YYYY)
evidence of your vaccination through a vaccine certification issued from the country where you were vaccinated, or an equivalent documentation. This documentation must contain the following information:
your name (first and last)
the name(s) of the vaccines you received
the country where the vaccine(s) were administered
the date the vaccine(s) were administered.
Your vaccination record and evidence above must be in English or translated into English prior to applying.

The following information is not mandatory but will help us process your application:

your National Health Index (NHI) number
your last address in New Zealand.

2.
If you’ve received acknowledgment that your overseas vaccinations have been added to the CIR you will be able to generate a My Vaccine Pass from Thursday 2 December.

The eight vaccines accepted for My Vaccine Pass, approved by the COVID Vaccination Technical Advisory Group, are: 

Pfizer/BioNTech
Janssen (Johnson & Johnson)
AstraZeneca (Oxford) 
AstraZeneca/Covishield (Serum Institute of India) 
Moderna
Sinopharm 
Sinovac (CoronaVac)
Covaxin (Bharat Biotech).

我注册了可是没有地方可以上传我的疫苗记录，也没有国际的电话可以打，发邮件也不回

不着急，人工处理的，5个工作日正常。

我新家跟市政要垃圾桶告我up to 14个工作日

我理解你的心情，但新西兰政府做事的效率就是这么低下，现在除了等，可能并没有什么更好的办法。希望你能够尽快得到回复

找到了邮件和解释，准备去办理，就是不知道哪里可以翻译

不会这么快回你的啊，你就按照下面步骤申请然后等待就行了，要14个工作日，我上周帮我妈申请的，昨天收到一封信，说正在处理中，在等待的这段时间给我一个Temporary Exemption，我觉得还是挺人性化的，虽然我们也不出去。

你如果时间上等得起的话，可以在邮件里问他们是否承认自己翻译的。保险起见可以找公证处。还有就是可以找一下新西兰政府接受的翻译机构，我以前移民的时候找到过一个internal affairs承认的翻译机构名单，不过这些机构在新西兰，所以价格可能有点小贵
