1. Photocopy of XXX original police clearance certificate letter issued by the local Public Security Bureau or Vice Squad which is less than 6 months old with official English translation. Please note that a police clearance certificate letter issued by an applicant's work unit, work unit security department, street office or talent centre is not acceptable
English translations must:
1. be full, complete and paid for by the applicant,
2. be prepared and certified by a translator,
3. not be prepared by an applicant, any member of their family or an immigration adviser assisting with the application,
4. on official letterhead, and
5. bears the stamp and signature of the translator.
2. Completed National Security Check form as enclosed for XXX. Please see attachment
3. Completed Residence application form section C1, E1, G5, G6 and H1 as highlighted. Please see attachment
Please write 'Not Applicable' to questions that do not apply to you.
4. Completed Medical Certificate section A2 as highlighted. Please see attachment
